Try to help people understand that it is no use trying to blame someone, or figuring out "whose fault" is was, but instead to look for deeper, often systemic, causes, and how to avoid them in the future.</p><p>The premise being, it probably made sense to the people at the time, to do what they were doing. Rather ask, "why" were they led to believe it was the right thing to do.
